New Hire Forms
Congratulations on your new job! We are excited for you to join our team at Duluth Public Schools.
If you have been offered a position, you will need a printer to complete the New Hire paperwork. If you do not have a printer, please come to the Human Resources Department and complete these forms. Please type in the information online before printing the New Hire Forms, you can sign the pages after they have been completed and printed.
Once you have completed the new hire paperwork and online I-9 form, please come to the Human Resources Department at the United Healthcare Building at 4316 Rice Lake Road, Suite 108, Duluth, MN 55811 with a voided check for the mandatory direct deposit and valid identification to complete the I-9 process. We will take your picture at that time for your District ID badge, if needing a badge.

Please complete the I-9 form and bring current valid documentation that establishes identity and employment authorization to the Human Resources Department to verify. Most common documents used are driver's license/photo ID and original Social Security Card or a Passport/Passport Card.
