Please read all the information on this page before completing and submitting your On Line Enrollment Form (below).
Open enrollment is your ONLY opportunity to make changes to your health insurance unless you experience a life changing/qualifying event. If you experience a life changing/qualifying event, you have 30 days to make changes.
This open enrollment is only for health plan changes. A separate open enrollment will be held in the fall to make changes to Dental, Flex Plan, Life, and Long Term Disability.
If you are not making any changes to your health plan, no action is required on your part.
There are NO changes to the PEIP health insurance plan design, co-payments, deductibles, etc. this year. There were changes to the cost levels on several providers. Please review the clinic listing to view the current cost levels for your network provider and clinic. PLEASE REVIEW THE UPDATED RATE SHEET WHICH REFLECTS THE NEW PREMIUM RATES.
As a reminder, changing your primary care clinic only is NOT a part of open enrollment. You can change your Primary Care Clinic (PCC) throughout the year by calling the toll-free number on the back of your health insurance card. If you call before the 15th of the month, it will usually be effective the first day of the following month. Therefore, if you only want to change your primary care clinic (and keep the same network i.e. BCBS, Health Partners, or Preferred One) effective July 1, 2017, you will need to call your network prior to June 15, 2017. Please clarify the effective date of the change while you are on the phone with your network.
However, if you are changing networks (Blue Cross Blue Shield, Health Partners, or Preferred One) or adding dependents, you will need to complete the On Line Enrollment Form (below). You will need to choose the network you are changing TO and will also need to fill in the clinic name and clinic number as provided on the attached clinic listing. Each family member may choose a different primary care clinic; all family members must use the same network.
The privacy and security of personal information is of utmost importance to Duluth Public Schools. Pursuant to our standard protocol, personal information of members of our District community is secured and not accessible or viewable by unauthorized third parties. We have eliminated the use of providing the employee social security number on the On Line Enrollment Form, and will use your employee ID number for identification purposes. However, you will need to provide any dependent’s social security numbers in order to add or make changes to their health insurance. Please be assured that this website is secured.
If you want to cancel or drop dependents from your health plan, you will need to contact the Benefits office via email firstname.lastname@example.org or call 336-8723. You are not able to cancel or drop your dependent(s) health insurance via the On Line Enrollment Form.
